We are using Access Care Planning to manage your electronic care records. We trust AccessUK with this because they are leading in the care industry with their technological innovations and we have worked together since 2015. You, your next of kin or health professional can have access to your electronic records simply by requesting this from your care office. You will need to share an email address and an account will be created for you to access your care plan, risk assessment, live daily visit notes and medication chart (as applicable).
